This page is intended to assist RTWC providers, clients, and family/caregivers with matters such as stepdown care to outpatient therapy, alternatives to PHP/IOP care, care coordination, court services (such as guardianship/conservatorship evaluations, mental health based criminal mitigation, family law evaluations, etc.).
We have offices in Cobb County, DeKalb County, and Gwinnett County, and work with clients virtually across Georgia.

We are not a primary provider for active psychosis or unmanaged schizophrenia; appropriate referrals will be provided when needed.

Navigation of mental health systems
Care coordination is a structured support service designed to help clients and families navigate the often complex mental health system with clarity and confidence. Rather than providing therapy itself, care coordination focuses on organizing, connecting, and aligning the various pieces of a client’s care so that treatment is consistent, appropriate, and effective. The caregiver is the designated client in this scenario.
By GAFC partnering with Red Top Wellness Center, care coordination may include:
Identifying appropriate levels of care (e.g., outpatient, IOP, PHP, inpatient)
This process is strengthened by our established network of trusted providers and programs. We are able to pair clients with vetted psychiatrists, specialty clinicians, and higher levels of care that align with their specific needs - reducing guesswork and accelerating access to quality services.
Connecting clients with trusted providers such as psychiatrists, specialty programs, or higher levels of care
Communicating with existing providers (with consent) to ensure continuity and alignment in treatment planning
Assisting families and caregivers in understanding diagnoses, recommendations, and next steps
Supporting transitions between levels of care (e.g., hospital discharge → step-down services)
Helping reduce gaps, confusion, and delays in accessing services
This service is especially valuable for families managing complex or evolving situations, individuals stepping down from higher levels of care, or clients who feel overwhelmed trying to navigate systems on their own.
The goal of care coordination is simple: to ensure that clients are not just receiving services - but receiving the right services, at the right time, with a clear and supported path forward.

At Greater Atlanta Family Counseling and Red Top Mountain Wellness Center, our Peer Specialists and Health & Recovery Coaches provide practical, experience-informed support to help individuals move forward with confidence, stability, and independence. Drawing from lived experience and structured training, peer support professionals offer a unique blend of empathy, accountability, and real-world guidance that complements clinical care. Our CPS and Coach, Brandon Lanus, will meet individuals in the community to assist with various goals.
Our services incorporate psychosocial rehabilitation (PSR), focusing on helping individuals build and restore the skills needed for daily living, social functioning, and community integration. This may include support with routines, communication skills, emotional regulation, problem-solving, and navigating work, school, or family responsibilities. The goal is not just symptom reduction, but meaningful improvement in day-to-day functioning and quality of life.
Peer Specialists and Recovery Coaches work alongside clients to develop personalized goals, strengthen coping strategies, and maintain momentum in recovery. Services may include recovery planning, care coordination, community linkage, skill-building, and ongoing support between sessions. They often serve as a bridge—helping clients apply what they are learning in therapy to real-world situations while reinforcing motivation and follow-through.
This service is especially valuable for individuals stepping down from higher levels of care, those needing additional structure and accountability, or anyone working toward greater independence and stability. By integrating peer support and psychosocial rehabilitation into our continuum of care, we enhance engagement, reduce relapse risk, and promote long-term, sustainable wellness.
