Care Coordination

Our care coordination program works with family members and caregivers who are responsible for the care of a person living with a serious or potentially serious mental health, developmental or intellectual disability, or addiction issue.
Because access to care is so challenging, it is critical that family members and caregivers understand the various resources and options available based on their circumstances.
Navigating the mental health system in Georgia can easily become overwhelming. Clients have often told us that they do not feel that they are given enough information by their psychiatrists or other institutions that they have been involved with.
Many of our providers are experienced social workers and clinicians who have extensive knowledge regarding resources and options for different types of mental health and substance abuse issues, and who will happily work with caregivers to ensure that they have a thorough understanding of the resources available.
We work closely with the caregivers to understand the needs of the individual, make sure that the caregivers has a thorough understanding of the diagnosis and prognosis, and collaborate with the caregivers to identify resources and ultimately assist in creating a plan for the caregiver to pursue.

Greater Atlanta Family Counseling typically does NOT have the resources to work with a person who is suspected or confirmed to be living with a severe and persistent mental illness. This service is exclusively for caregivers.
While the primary objective is to assist caregivers to identify resources and create a plan, we CAN provide therapy to the caregivers to assist in stress management and caregiver burnout.

Some of our social workers have completed the SOAR Certification, a training that is designed to assist persons living with SPMI and at-risk for homelessness in obtaining social security benefits. We also can generally assist in understanding if a party would qualify for benefits and how they would obtain their benefits.
