Care Coordination

Our agency understands that navigating the mental health system in Georgia can be an overwhelming task, particularly for people who are inexperienced and/or people who have limited financial resources.

Georgia is currently ranked 48th out of 50 states in the country in regards to “Access to Mental Health Care.” This is mostly referencing a trend where many mental health practitioners in Georgia refuse to participate with health insurance companies for various reasons, and insist on being paid their full hourly rate by the consumer.

Because access to care is so challenging, it is critical that consumers - particularly people just entering the mental health system - understand the various resources and options available based on their circumstances.

In response to this, Greater Atlanta Family Counseling offers Care Coordination services.

Our care coordination program works with family members and caregivers* who are responsible for the care of a person living with a serious or potentially serious mental health or addiction issue.

*Please remember that our agency does not currently have the infrastructure to work with clients who need more than 1-2 hours of therapy a week or who have a psychiatric condition where the severity is out of our scope of practice, thus, we work strictly with the caregivers in this service.

Clients who pursue care coordination will be placed with a social worker who is intimately familiar with the mental health system in metro-Atlanta and North Georgia. We work closely with the clients to understand the needs of the individual, make sure that the caregivers have a thorough understanding of the diagnosis and prognosis, and collaborate with the client to identify resources and ultimately create a plan for the caregiver to pursue that is the most clinically appropriate.

We also work with the caregivers to manage their own stress and process the challenges of adjusting to a loved one struggling with mental health or addiction issues.

If you believe that you could benefit by working with one of our clinicians in this capacity, please email info@gafamilycounseling.com and reference care coordination, or you can call us to schedule an appointment at 770-421-5271.
